Additional problems may include:
- the staff who should be their own quality controllers can readily off-load responsibility onto inspectors and fail to check their own work.
- inspection itself however is poor at spotting failures. Only samples are taken. Not every defective item can be identified. The whole batch (sample failure) will have to be intercepted and scrapped or re-worked.
- Some components may be invisible (they lie inside others). This is very typical of aircraft or nuclear installations.
